New Patient Evaluation
In-Person
Required for: first-time patients in Utah's medical cannabis program
Initial evaluation, clinical history review, and certification if appropriate. Required by Utah law for new patient registration.
What's included
- Clinical evaluation for medical cannabis appropriateness
- 6-month medical cannabis card if certified
- On-site EVS submission
- Time for your questions and treatment guidance
- Card issued is valid for 6 months
- To continue beyond 6 months, schedule the 6-Month Follow-up visit

Required for: all first-time patients to extend their card beyond the initial 6 months
A quick check-in to see how things are going. Is the program helping? Any questions about your treatment? We'd rather hear from you at 6 months than have you wait a full year if something isn't working.

- Time for your questions about the program

- Required to keep your card active beyond the first 6 months

Annual Renewal

Required for: returning patients renewing their medical cannabis card after the first year
Standard annual renewal to keep your card active. After your first year (new patient evaluation + 6-month follow-up), all future renewals are annual.
