New Patient Evaluation

For patients getting their first Utah medical cannabis card.

In-Person

6-month card

What's included

• Clinical evaluation and review of your medical history
• Discussion of your symptoms and qualifying conditions
• Medical cannabis certification if appropriate
• On-site EVS submission
• Same-day card printed and laminated in our office
• Walkthrough for Utah ID and EVS setup if you need it

• After this visit, you'll return for a 6-Month Follow-up to extend your card to a full year

Already have a Utah card

Transfer Patient Evaluation

For patients switching to Solenta Wellness from another Recommending Medical Provider.

In-Person

12-month card

What's included

• Clinical evaluation and review of your medical history
• Transfer paperwork handled for you
• EVS updated to list Adam as your Recommending Medical Provider
• Same-day 12-month card printed and laminated in our office

• Because you're already active in Utah's program, you receive a full 12-month card right away and continue directly to annual renewals — no 6-Month Follow-up needed

New patients · first year

6-Month Follow-up

Telehealth

Required for new patients to extend the first card beyond 6 months. Not required for transfer patients — they receive a 12-month card at their transfer evaluation.

Available by telehealth anywhere in Utah. A quick check-in to see how things are going. We'd rather hear from you at 6 months than have you wait a full year if something isn't working.

What's included

• Brief check-in on treatment response and any side effects
• Time for your questions about the program
• 1-year medical cannabis card extension
• On-site EVS update

• New-patient path only — transfer patients skip this step

Existing patients

Annual Renewal

Telehealth

Required for: Returning patients to renew their annual card.

Available by telehealth from anywhere in Utah. Standard annual renewal to keep your card active. After your first year, all future renewals are annual.

• Brief medical history review
• Treatment response check-in
• 12-month medical cannabis card renewal
• On-site EVS update
• Time for your questions

• For patients past their first card. New patients renew here after completing their 6-Month Follow-up. Transfer patients renew here annually starting one year after their transfer visit.
