Book your visit
Choose a date and time that works for you.

Create your Utah ID account (new patients only)
Required for the state's Electronic Verification System. Set this up at medicalcannabis.utah.gov/patients/apply-for-patient-card (opens in a new tab) before your appointment.

Meet your provider
Your evaluation will determine if medical cannabis fits your situation.

Get your cannabis card
Once your provider approves your application in EVS, the state typically issues your card the same day.

Visit a medical cannabis pharmacy
Choose products with a Pharmacy Medical Provider (PMP) at any Utah-licensed cannabis pharmacy.
Eligibility
Utah's Qualifying Conditions
Utah law recognizes the following conditions as qualifying for medical cannabis. If you have one of these conditions, you may be eligible for a recommendation.
- Acute pain for a condition such as surgery
- Alzheimer's disease
- Amyotrophic lateral sclerosis (ALS)
- Autism
- Cachexia
- Cancer
- Crohn's disease or ulcerative colitis
- Epilepsy or seizures
- HIV or AIDS
- Multiple sclerosis (MS) or muscle spasms
- Nausea that's not caused by pregnancy, cannabis-induced cyclical vomiting syndrome, or cannabinoid hyperemesis syndrome
- Persistent pain that lasts longer than 2 weeks
- Post-traumatic stress disorder (PTSD) — also being treated by a mental health provider
- A rare condition or disease defined by the National Institutes of Health
- A terminal illness or hospice care with a life expectancy of less than 6 months
